Did you work the first 4 shifts in the bar? You need to be at the point where you can interact with the patrons.
You have to work at the bar more, I'm not sure exactly how many times.
I suppose this is to make it look like you're regular staff, not just a newbie of 1-2 shifts immediately coming over and talking to the spy.

It also depends on which version you have. The current official public release doesn't have it, the one in the first post doesn't (neither the most recent in the first post or the "stable release" one), ones that are leaked from other sources or in a post then it might be.
So the Bus Scenes happening seems dependent on your Desire. Higher Desire = more likely.
When you do the male origin it can get high enough easily, but if you do female origin you're capped at like 200 or 250 and there's pretty much 0 chance of it happening unless you use Lebo.
I feel like this needs to be adjusted. The busses are supposed to be dangerous, but when you do a female origin it just doesn't feel that way until you start going with Robin. Then, it suddenly happens like every time.

And tying the non-consensual scenes with desire doesn't really make much sense.

Odd that. I started a new female origin game yesterday. Literally the first time alone on the bus – returning from the beach after going with Emile – Sammy got poked (and pregnant).
This was on the current release candidate, but as far as I know no work has been done on the bus since at least the last public release.
If you want danger from day one, you also need to turn off Danger Limit. Otherwise the danger starts at 0 and ramps up over a few weeks.

Did that got fixed? Last i heard if you turned it off right at the start it would stay at the number it was at when you turned it off, being zero.
It is news to me that is was ever broken. Mind you, the oldest version that I have access to that even has that setting is 0.3.3.27; The v21 you can find here only has the Danger Days setting. Before that setting was exposed, I believe you were locked into that slow ramp-up unless you mucked around with game files and/or console commands.
To recap:
Danger Days is the on/off switch for all of that content; Defaults to off.
Danger Limit decides whether danger is immediate or starts slow; Defaults to slow, and obviously does nothing if Danger Days is turned off.
Beyond those two settings, the choice of Origin Perk has a considerable impact on how much trouble Sammy will get into. I never have any trouble finding trouble, if finding trouble is what I am after.

I'm using the version from this thread, 0.3.3.21, and there is no "Danger Limit" setting in the Cheat menu.
Still though, if you do Male origin start, then the bus scenes will happen very close to the start of the game since your desire will be over 300 most of the time, because Male origin adds +300 to max desire.
Also, if you do female origin with Bimbo and lose your virginity, then your Max Desire will be 300, so there will be a decent chance of bus scenes happening at max (300) desire. Add some alcohol and the max can get up to 340 for even better chances.
If you don't have the Bimbo perk or Male origin perk, then your best bet to get bus scenes is to use Lebo.
Succubus and Broken Virgin perks will also increase Max Desire and boost chances of bus scenes, but you can't get those perks right from the beginning of the game.

EDIT: After a few weeks the bus scenes do happen at lower desire levels. So, the danger limit setting would fix this.
